The idea

A UK supported-living provider wanted every care assessment to become a care plan in the same clear structure, whoever completed it. Assessments cover health, daily living, risks and preferences, and every plan has to capture them correctly before staff can rely on it.

The client is a UK supported-living provider. At this stage the work is a prototype, so the provider is not named.

The problem

Turn a long assessment into a consistent, reviewable care plan.

  3. UK details, such as NHS numbers and advance-care documents, must be captured correctly.

What we built

A structured assessment, a three-stage AI pipeline and a printable plan.
